A Safe Haven Foundation in Chicago seeks a housing case manager to help clients secure stable housing through Rapid Rehousing and Housing First programs. You will assess needs, coordinate with landlords, and guide clients through paperwork and eligibility.

The role requires strong collaboration, knowledge of HUD resources, and experience with vulnerable populations. This on-site position offers growth and the chance to make a lasting impact in the community.

Competenze

• Bachelor's degree or 1–2 years related experience or equivalent.
• 3+ years of case management experience with housing programs preferred.
• Knowledge of HUD programs and local housing resources.
• Computer skills: Excel, Word, Outlook; keyboarding.

Mansioni

• Works with families and individuals experiencing homelessness to establish housing goals and obtain independent housing.
• Assists clients with navigating the administrative processes and paperwork for housing.
• Establishes relationships with landlords and property management to identify housing opportunities.
• Maintains case management documentation and monitors client progress toward housing goals.
• Views and evaluates potential housing locations for suitability and maintenance.
• Travels as required.
• Performs other duties as assigned by management.
