Job summary

NeighborWorks Housing Solutions is seeking a compassionate Housing Stabilization Case Manager to support families transitioning from shelter to permanent housing. You will provide trauma-informed case management, help with housing search, and connect clients to employment and supports to sustain stability.

The role reports to the Stabilization Manager and works with HomeBase Case Managers and partners to deliver coordinated services and resources.

Qualifications

• Bachelor's degree preferred; relevant experience may be considered in lieu of degree.
• 2+ years of experience in housing stabilization, case management, or related work.
• Knowledge of Housing First and Rapid Re-Housing models preferred.
• Experience with recovery and/or survivors of domestic violence is strongly preferred.
• Strong interpersonal and communication skills; ability to build rapport with diverse populations.
• Valid driver’s license and access to reliable transportation.
• Bilingual in English and Spanish or Haitian Creole is a plus.

Responsibilities

• Provide client-centered, trauma-informed case management services.
• Uphold professionalism, ethics, boundaries, and confidentiality.
• Ensure adherence to program policies and Housing First principles.
• Assist clients with housing search and stabilization, including communicating with landlords and securing leases.
• Support moving into housing, including furniture and household items.
• Coordinate care with recovery programs, medical providers, housing authorities, and community organizations.
• Facilitate access to employment services, education, childcare, and public benefits.
• Promote housing retention through tenancy education, utility management, and timely rent payments.
• Conduct regular home visits and monitor progress.
• Develop individualized service plans with measurable, time-bound goals.
• Accompany clients to appointments and provide advocacy.
• Maintain documentation including assessments, notes, and funder reports.
• Participate in staff meetings, supervision, and trainings.
