Job summary

The Night Ministry in Chicago seeks an OHM Case Manager to conduct outreach in the community, engage individuals using trauma-informed approaches, and transition clients into intensive housing case management. You will complete housing assessments, assist with documentation, and coordinate with CHA programs.

Responsibilities include data entry in HMIS, building referral networks, and supporting housing placement efforts across partners, with varied hours to meet client needs.

Qualifications

• Bachelor's degree in social service or related field from an accredited college or university.
• Two to five years’ experience in housing-focused case management, homelessness services, street outreach, or related field.
• Demonstrated experience using HMIS for data entry, reporting, and case documentation.
• Experience with the Coordinated Entry System (CES); Skilled Assessor status strongly preferred.
• Familiarity with Chicago Housing Authority (CHA) programs and housing processes strongly preferred.
• Must have a driver's license with a good driving record and ideally, access to car. Willingness and ability to travel locally.
• Strong verbal and writing abilities.

Responsibilities

• Conduct proactive street outreach through multiple programs and outreach venues; shifts may include evenings and weekends.
• Engage individuals on the street, build rapport using trauma-informed, harm-reduction approaches.
• Transition outreach engagement into intensive housing case management.
• Complete housing assessments, applications, and documentation for housing placement.
• Assist clients in obtaining identification, income, benefits, and other needed documents.
• Provide follow-up support and navigation through housing placement and stabilization.
• Conduct CES assessments and work toward Skilled Assessor status.
• Maintain knowledge of CHA processes, programs, and eligibility requirements.
• Collaborate with housing providers, shelters, healthcare providers, and community partners.
• Maintain compliant HMIS and other required systems; ensure audit-ready case files.
