Job summary

GMHC is seeking a Housing Case Manager in New York City to provide comprehensive case management for clients facing housing instability. You will conduct home visits, assess needs, develop housing plans, and coordinate with program partners to secure stable housing.

The role combines fieldwork across NYC with in-office duties and remote work as determined by supervision. Strong organizational and communication skills, and experience with housing resources are essential.

Qualifications

• High School Diploma or equivalent required; 2+ years helping individuals facing homelessness or housing instability preferred.
• Associate or Bachelor’s degree in Social Work, Human Services, or related field preferred.
• Experience with housing resources, entitlements, and case management strategies.

Responsibilities

• Maintain a caseload of up to 25 clients across NYC.
• Conduct at least one monthly home visit per client to assess stability.
• Coordinate housing options, applications, and lease negotiations.
• Escort clients to healthcare, entitlement, and housing appointments.
• Document progress and submit required reports in GMHC systems.
