Job summary

S:US is seeking a Case Manager for their homeless prevention program - Home Base in New York, NY 10033. You will perform assessments, develop service plans, connect clients to housing resources and benefits, and support long-term housing stability and self-sufficiency.

Qualifications

• HS Diploma, GED, or AA degree and two years qualifying experience.
• MSW or bachelor’s degree preferred.
• Knowledge of homelessness and related services may help.

Responsibilities

• Provide case management to individuals and families at risk of homelessness.
• Conduct assessments and develop individualized service plans.
• Connect participants to housing resources and benefits.
• Assist with financial assistance applications and enrollment.
• Advocate and coordinate services with providers and landlords.
• Maintain accurate case records and monitor progress.
