Job summary

The Salvation Army of the Twin Cities is seeking a Harvest Hills/HOPE Harbor Case Manager to provide holistic, person-centered case management for residents. The role focuses on housing stability, employment and personal development, with a strong emphasis on resident self-sufficiency and successful program completion.

Responsibilities include life skills facilitation, documentation, community outreach, and collaboration with property management.

Qualifications

• Bachelor's degree in a relevant field. A combination of education and experience may be considered in place of a degree.
• If the degree is in social work, state board licensure is required.
• Previous social services experience is required.
• Experience conducting long-term case management is preferred.
• Must successfully pass background checks, possess a valid driver's license, and meet MVR clearance standards.

Responsibilities

• Provide long-term strength-based, person-centered case management and develop service plans.
• Maintain accurate records in physical and electronic files and ensure documentation standards.
• Promote Harvest Hills program through community engagement and partnerships.
• Recruit eligible participants and conduct outreach to raise awareness of services.
• Facilitate life skills, financial literacy, and personal development sessions.
• Collaborate with property management to support resident tenancy.
• Support seasonal and special events aligned with program goals.
