Qualifications

• 2-3 years’ experience in providing services to individuals or families with behavioral health challenges.
• Experience in housing search and placement preferred.
• Education may be substituted with experience.

Responsibilities

• Conduct housing intake, search, and placements.
• Develop individualized housing search and placement plans.
• Support participants to navigate transportation needs.
• Document daily activities and tasks performed.
