Job summary

MedZed seeks a Housing Navigator II to support field-based housing placement and ongoing tenancy for members in the Los Angeles area. You will assess needs, craft service plans, link to resources (mental health, benefits, jobs), and provide crisis support through in-field visits and documentation in CRM systems.

Role emphasizes Housing First principles, trauma-informed care, and strong outreach to enroll new members while ensuring compliance with health plan guidelines and local regulations.

Qualifications

• High School Diploma or equivalent required.
• At least 1+ years in housing services, case management, homelessness services, or social services preferred.
• Knowledge of community resources and local housing options.
• Ability to travel locally and work in field-based settings.
• Strong documentation and compliance practices.

Responsibilities

• Manage housing caseload up to 55+ members.
• Conduct intake and develop individualized service plans.
• Provide emotional support and use motivational interviewing where appropriate.
• Assist with housing searches; advocate with landlords; conduct home visits.
• Accompany members to housing appointments and enroll new members.
• Conduct calls, field visits, and door-knock outreach.
• Build and maintain a member panel with consistent outreach.
• Coordinate referrals to health plans, providers, and community resources.
• Attend trainings and participate in team huddles; document timely notes in Salesforce Health Cloud.
• Ensure compliance with CalAIM, DHCS, and plan requirements.
