Qualifications

Responsibilities

• Manage a housing caseload of 55+ members.
• Conduct intake and develop individualized service plans.
• Assist with housing searches and advocate with landlords.
• Perform home visits, enroll new members, and coordinate referrals.
• Document case notes in Salesforce Health Cloud and portals.
• Attend trainings and participate in team huddles.
• Maintain compliance with CalAIM and health plan requirements.
