Job summary

The People Concern is hiring an ICMS Case Manager to support clients in achieving housing stability and self-sufficiency through evidence-based practices such as harm reduction, housing first, and trauma-informed care in a 100% onsite setting.

You will manage a caseload, collaborate with LA County DMH and DHS, and coordinate services to meet individualized needs. Strong drivers include independence, community integration, and ongoing support for recently housed clients.

Qualifications

• High School Diploma or GED or Equivalent is required.
• Valid CA Driver's license, reliable car, auto insurance, and acceptable driving record.
• Willing and able to transport Clients in your personal vehicle.
• Able to work in a high tolerance Harm Reduction model with Clients experiencing multiple barriers.

Responsibilities

• Maintain a caseload of approximately 20 high-acuity Clients for intensive case management services (up to 40 if low acuity).
• Develop relationships with Clients with a focus on facilitating independence and maintenance of improved physical and mental health.
• Collaborate with LA County DMH and DHS programs to enhance coordination within the Clients' treatment team.
• Engage Clients and implement interventions with harm reduction and trauma-informed care principles.
• Assist Clients in screening, intake, and enrollment including orientation to program policies and goals.
• Complete biopsychosocial assessments and functional needs assessments to guide treatment planning.
• Write quarterly collaborative goal plans to improve quality of life and health outcomes.
• Maintain accurate progress notes and data in electronic databases as required.
• Ensure Client documents are complete and filed according to guidelines.
• Maintain confidentiality of Client information per HIPAA.
• Assist Clients in applying for and obtaining benefits to which they are entitled.
• Coordinate appointments, transportation, and follow-up services to access health care and community resources.
• Transport Clients in personal vehicle to agencies and healthcare providers in the LA area.
• Support Clients by teaching life skills such as budgeting, housekeeping, meal prep, and appointment scheduling.
• Communicate with property managers and third-party partners.
• Work with a multidisciplinary team including supervision and trainings.
