Job summary

Ruby's Place dba Restorative Pathways in Tracy, CA seeks a full-time Case Manager for its 150-bed shelter program. The role focuses on direct client services, assessments, and housing navigation to support residents toward stable, permanent housing.

You will work within a multidisciplinary team, handle a high caseload, and uphold trauma-informed practices. Qualifications include a bachelor’s degree in social work or related field, 2+ years of case management experience, knowledge of housing

Qualifications

• Bachelor’s degree in social work, human services, or related field preferred; equivalent experience considered.
• Minimum two years of case management or direct services to individuals experiencing homelessness or housing instability.
• Experience in shelter or residential settings preferred.
• Knowledge of housing resources, benefits systems, and community services.
• Strong organizational, communication, and engagement skills.
• Ability to manage a high caseload in a fast-paced environment.
• Experience with HMIS or similar data management systems.
• Knowledge of trauma-informed care, harm reduction, and housing-first approaches.
• Experience working with individuals with complex needs.
• Valid CA driver’s license, proof of insurance, and driver approval to operate agency vehicles.

Responsibilities

• Engage participants to assess needs, strengths, and barriers to housing stability.
• Complete intake assessments and gather housing history, income, and service needs.
• Build and maintain trusting relationships using a trauma-informed approach.
• Develop individualized service plans focused on housing stability and resources.
• Provide ongoing case management through regular meetings and goal tracking.
• Assist with identification, benefits, employment resources, and other services.
• Identify housing opportunities and coordinate referrals to programs and resources.
• Support housing applications, documentation, and preparation for placement.
• Coordinate with housing navigators, landlords, and partners to secure housing.
• Collaborate with Residential Advocates, Program Coordinator, and leadership for coordinated care.
• Participate in case conferences and team meetings to discuss progress.
• Maintain accurate case notes, service plans, and HMIS data entries.
• Advocate for participants to access services and resources.
• Complete timesheets on time, participate in supervision, and document procedures.
• Other duties as assigned.
• Local travel may be required.
