Job summary

Lutheran Community Services NW is seeking a Health-Related Social Needs (HRSN) Case Manager – Houselessness Prevention to serve individuals and families at risk of houselessness. You will conduct screenings, create care plans, and coordinate housing and health resources to improve housing stability and well-being.

Qualifications

• Bachelor’s degree in social work, psychology or counseling or at least two years of experience with high-needs families.
• Must have well-developed communication skills, both written and verbal.
• Ability to work under pressure in a multicultural setting and maintain confidentiality.

Responsibilities

• Conduct HRSN screenings and housing stability assessments.
• Develop individualized care plans for housing stabilization and health needs.
• Assist clients with identifying safe and stable housing options.
• Provide financial support for rent and related housing expenses using HRSN funding.
• Provide ongoing case management and stabilization support.
• Coordinate referrals to food resources, transportation, income support, and healthcare services.
• Liaise with landlords, housing providers, and legal resources to support housing retention.
