Job summary

The Salvation Army Northern Division Harbor Light Center (HLC) in Minneapolis is seeking a Case Manager to provide individualized supports to adults pursuing housing, employment, and self-sufficiency goals. You will maintain participant records, recruit eligible participants, and promote social services programs within a compassionate, person-centered framework.

Qualifications

• Bachelor’s degree in a relevant field; combination of education and experience may be considered.
• State board licensure is required if the degree is in social work.
• Previous experience in social services is required.
• Experience with long-term case management is preferred.
• Must successfully pass all required background checks.
• A valid driver’s license is preferred, and candidates must meet the MVR requirements.

Responsibilities

• Provide short- and long-term, strength-based, person-centered, holistic case management.
• Develop and implement individualized service plans, monitor progress, and support housing, employment, and self-sufficiency goals.
• Maintain accurate participant records in case files and databases.
• Recruit eligible participants for housing programs through outreach and referrals.
• Promote social services and corps programs to increase engagement and accessibility.
• Collaborate with colleagues and participate in community events, forums, and outreach activities.
• Facilitate educational opportunities and develop a catalog of local resources.
