Job summary

The Salvation Army Northern Division Anoka 360 Life Center in Blaine, MN, is seeking a Case Manager to deliver strength-based, person-centered case management to individuals and families facing homelessness or housing instability. You will help participants identify needs, navigate resources, and develop housing stability plans while coordinating services.

• Bachelor’s degree in a relevant field; equivalent experience may be considered.
• State social work license required if the degree is in social work.
• Previous experience in social services required.
• Must pass background checks and have a valid driver's license.
• Strong interpersonal, communication, and organizational skills.

Responsibilities

• Provide direct housing assistance and help participants access resources.
• Deliver strength-based, person-centered case management and develop housing stability plans.
• Complete participant assessments and maintain program documentation.
• Develop and maintain community partnerships and coordinate referrals.
• Assist with seasonal and special events.
