Job summary

The Salvation Army Northern Division Anoka 360 Life Center in Blaine, MN is seeking a Case Manager to provide strength-based, person-centered services for housing stability. You will guide participants to access housing resources, develop goals, and coordinate supports with community partners.

Responsibilities include direct client assistance, completing assessments, maintaining documentation, and contributing to outreach while meeting program requirements.

Qualifications

• Bachelor’s degree in a relevant field; equivalent combinations may be considered.
• Must be licensed by the state board if the degree is in social work.
• Previous experience in social services required.
• Must pass background checks and possess a valid driver’s license.
• Strong interpersonal and communication skills.
• Maintains confidentiality and exercises sound judgment when handling sensitive information.
• Works with integrity, valuing honesty and diligence.
• Excellent organizational skills and attention to detail.
• Proficient with PC, Microsoft Office, databases.

Responsibilities

• Provide direct housing instability assistance and resource navigation.
• Deliver strength-based case management and develop housing stability plans.
• Maintain accurate case files and required program documentation.
• Develop partnerships, coordinate referrals, and connect clients to resources.
• Assist with seasonal and special events.
