Qualifikationen

• Bachelor’s degree in a relevant field; equivalent combo may be considered.
• Must be licensed by the state board if the degree is in social work.
• Prior experience in social services required.
• Long-term case management experience preferred.
• Must pass background checks and have a valid driver’s license with MVR requirements.

Aufgaben

• Provide strength-based, person-centered case management to individuals and families experiencing housing instability.
• Identify needs, navigate housing and community resources, and coordinate referrals.
• Complete assessments, develop housing stability plans, and document findings.
• Collaborate with community partners to secure housing resources and supports.
• Maintain accurate case files and program documentation in the system.
