Job summary

Requires an Associate’s Degree and 2+ years of direct service experience, plus a valid driver’s license. Full-time role with benefits and a 40-hour workweek.

• 2+ years relevant experience in direct service and case management.
• Valid Driver’s License required.

Responsibilities

• Provide home- and community-based case management to homeless women and families in Bridgeport, CT.
• Conduct intake assessments and develop individual service plans for housing stability.
• Coordinate with landlords, treatment providers and community resources; assist with transportation and money management.
• Maintain caseload of 8–12 cases; document data entry and progress notes; support reunification of families.
