Job summary

The Connection is seeking a Case Manager 2 to provide home- and community-based case management to homeless young adults across Connecticut. You will conduct intake assessments, assist with locating apartments, and develop individual service plans to promote self-sufficiency.

You will liaison with treatment providers, provide transportation, build money and employment readiness skills, and support tenancy while maintaining data entry and documentation.

Qualifications

• Associate’s Degree required.
• 2+ years relevant experience in direct service and case management field.
• Valid Connecticut Driver’s License.
• Regular and predictable attendance is required.

Program Summary:

The Start Program is a nationally recognized program that works with homeless and unstably-housed youth, age 18-24, located throughout the state of CT through an innovating service delivery model that combines short-term rental assistance with intensive, community-based case management services targeted towards housing stabilization. Through a lens of trauma-informed care and positive youth development, the program partners with young adult participants to identify goals and resources that help them to exit homelessness and achieve long-term stability.

Position Summary:

Working within our Young Adult Services, the Case Manager 2 works with the START program staff to provide services to stabilize and reunite families experiencing complex issues such as substance use, behavioral health matters, and homelessness.

• Regular and predictable attendance is required

Orientation:

If selected for employment with The Connection, newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
