Job summary

The Connection in Bridgeport, CT is seeking a Case Manager 2 to deliver comprehensive case management services within the SHP program, focusing on housing stabilization for chronically homeless women and families in Bridgeport. You will conduct home- and community-based assessments, coordinate resources, and support participants toward stable housing.

Location: Bridgeport

Salary: $22.00/hour

Schedule: Full time, 40 hours, 1st shift, Monday-Friday

Program Summary

The Supportive Housing Program (SHP) of Fairfield works with chronically homeless women and families in Bridgeport, CT through. The program offers permanent rent subsidy and intensive, community-based case management services targeted towards housing stabilization. Through a lens of trauma-informed care, the program partners with program participants to identify goals and resources that help them achieve long-term stability.

Working within our Homeless Services, the Case Manager 2 works with the SHP program staff to provide services to stabilize and reunite families experiencing complex issues such as substance use, behavioral health matters, and homelessness.

• Valid Driver’s License
• Regular and predictable attendance is required

Orientation

If selected for employment with The Connection, newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscomment office located in Middletown.
