Job summary

The Connection Inc. in Bridgeport, CT is seeking a Case Manager 2 to provide home- and community-based case management to homeless women and families. You will develop service plans, coordinate with landlords and providers, and support housing stability for clients.

Requires an Associate’s Degree and 2+ years of direct service experience, plus a valid driver’s license. Full-time role with benefits and a 40-hour workweek.

Responsibilities

Program: SHPPosition: Case Manager 2Location: BridgeportSalary: $22.00/hourSchedule: Full time, 40 hours, 1st shift, Monday-FridayProgram Summary:The Supportive Housing Program (SHP) of Fairfield works with chronically homeless women and families in Bridgeport, CT through. The program offers permanent rent subsidy and intensive, community-based case management services targeted towards housing stabilization. Through a lens of trauma-informed care, the program partners with program participants to identify goals and resources that help them achieve long-term stability.Position Summary:The Case Manager 2 is responsible for providing home- and community-based case management services to homeless women and families throughout Bridgeport, CT.
