Job summary

The Connection in New Haven, CT is hiring a Case Manager 2 to provide staff support and direct client care within the REACH program. You will perform intake assessments, develop and implement individual care plans, and liaise with treatment providers and community resources to assist adults re-entering the community after incarceration.

This full-time, first-shift position requires an Associate’s Degree and at least two years of related experience in case management, plus a valid CT driver's

Qualifications

• 2 years of related experience in direct service and case management
• Associate’s Degree or higher (education requirement)
• Valid Connecticut driver’s license
• Regular and predictable attendance is required

Responsibilities

• Provide staff support and direct client care within the program
• Deliver comprehensive case management services including intake assessments and care plan development
• Liaise with treatment providers and community resources to support re-entry
• Collaborate with Community Justice program staff to rehabilitate and guide clients back into the community
