Job summary

Responsibilities include housing navigation, landlord outreach, coordinating with CES matches, and documenting in HMIS/CES/WellSky.

Qualifications

• Bachelor's in social work or equivalent degree (preferred).
• Minimum 1-year experience in case management working with people in recovery, mentally ill and/or homeless (required).
• 1-year experience with HMIS or WellSky (preferred).
• Experience with tenant/landlord relationships preferred.
• Must have driving license (driving required).

Responsibilities

• Provide holistic, harm-reduction case management for housing stability.
• Visit residents and develop case plans; maintain case load.
• Coordinate with landlords, housing authorities, and social services to secure housing.
• Enter resident case notes into HMIS/CES/WellSky; ensure timely documentation.
• Deliver life skills classes and connect residents to resources.
