Job summary

Ampla Health is seeking a Housing Case Manager to provide intensive case management and crisis intervention for clients affected by HIV/AIDS. You will assess eligibility, develop quarterly service plans, and connect clients with housing and supportive services to prevent homelessness.

Collaborate with medical and social service providers, document outcomes, prepare invoices, and ensure compliance with grants and program requirements. Strong communication and organizational skills are essential.

Qualifications

• High School Diploma required.
• Medical Assistant preferred.
• Proficient in Microsoft Office; typing speed 40 wpm.
• Strong communication, conflict resolution, and problem-solving skills.
