System One, Inc. is seeking a dedicated Case Manager to support homeless individuals and families in a Safe Parking Program.

Under the guidance of the Interim Housing Senior Manager, you will conduct outreach, assessments, develop Housing & Services Plans, assist with entitlement applications, and coordinate housing research and referrals. You will maintain confidentiality, manage a caseload, document in HMIS within 24 hours of engagement, and collaborate with community partners to promote

• Associate’s degree or higher in social services or a related field.
• 2+ years of job-related experience with homeless individuals/families.
• Experience in case management and homeless programs preferred.
• Strong organizational skills and attention to detail; ability to manage multiple tasks.
• Proficiency with HMIS or similar systems; knowledge of community resources.

Responsabilidades

• Maintain confidentiality in all matters pertaining to the participants.
• Develop effective and trusting relationships with individuals/families to facilitate services.
• Maintain the assigned caseload of participants.
• Assist participants in completing Housing & Services Plan and Budget within three days of enrollment.
• Meet with participants twice a month to develop/update the housing service plan and identify barriers.
• Complete intakes and provide orientation, including program guidelines and scheduling.
• Maintain accurate client data in HMIS and documentation in client files.
• Submit case notes into HMIS within 24 hours of engagement using DAP format.
• Update information and referrals, and complete case notes within 24 hours after services.
• Conduct daily/weekly wellness checks and safety inspections of the facility/premises.
