Community of Hope in Washington, DC is seeking a full-time Housing Stability Case Manager for our Permanent Supportive Housing, Site Based Program. This role helps families and individuals secure and maintain stable housing, increase self-sufficiency through income, benefits, and education, and strengthen overall family stability.

Based at Mississippi Ave in Southeast, DC, you will work within a housing-first framework, coordinate with community resources, develop action-oriented goal plans, and

Qualifications

• Bachelor’s degree in social work or related field required.
• Minimum one year work experience in social service or related field.
• Strong interpersonal, conflict-resolution, and organizational skills required.
• Ability to work independently and as a team.
• A valid driver’s license and regular access to a car, as well as proof of auto insurance, required for staff working in scattered site housing programs.

Responsabilités

• Welcomes adults and families to the program and reviews program rules.
• Develops measurable goal plans with action steps and updates as needed.
• Networks with community resources and tracks client participation.
• Assists with housing search and landlord communications as needed.
• Conducts home visits and provides tenant education as required.
• Maintains regular client communication and documents engagement efforts.
• Works within a housing-first, client-centered framework.
