Job summary

Claratel Behavioral Health is seeking a Housing Support Case Manager to assist adults in permanent subsidized housing, promoting stability, wellness, independence, recovery, and community integration. You will carry a caseload of 25-30 clients, provide care coordination, document thoroughly, and connect clients to internal/external services while supporting IRP goals and housing stability.

Requires a bachelor’s degree and at least one year in behavioral health case management; in‑person work

Qualifications

• Bachelor’s degree in related field or equivalent education and experience.
• One year of experience in a behavioral health setting providing case management/community support service or similar.
• Proficient technology skills to use EMR, telehealth systems, and cell phone.

Responsibilities

• Carry a caseload (25-30 clients) and conduct a minimum of 1 visit per week per client within residence or community setting for the first three months; after three months, twice monthly.
• Provide care coordination and linkages to internal/external services to assure continuum of care.
• Provide ongoing case management and skills training aligned with treatment plans to achieve goals.
• Gather information from clients and support systems to ensure appropriate level of care.
• Recognize the need for crisis intervention and notify supervisor/clinical contact of escalated situations.
• Complete timely documentation for all service contacts (within 24 hours).
• Participate in weekly team meetings to discuss client treatment needs.
• Meet agency productivity standard of 40 billable hours per month.
• Regularly meet with program manager for professional growth and performance evaluation.
• Perform additional duties as assigned.
