Job summary

Claratel Behavioral Health is hiring a Case Manager for the Project HEAL program to coordinate transitions from hospital care into Transitional Housing and outpatient services. Qualifications

• High school diploma or GED required.
• At least 1 year of experience in behavioral health case management.
• Must possess basic computer and typing skills.
• Care coordination skills to assist individuals in accessing resources.
• Must have transportation to and from work.

Responsibilities

• Coordinate discharge planning with hospital or acute care providers and medical home/home health services for identified individuals.
• Conduct transition planning to support movement from facility-based care into residential settings.
• Facilitate enrollment and linkage to outpatient services including counseling, psychiatric services, and substance use treatment.
• Coordinate care with residential providers, medical providers, behavioral health clinicians and community services agencies.
• Assist participants with appointment scheduling, transportation planning, benefits linkage, and service navigations.
• Monitor engagement in treatment and supportive services into housing.
• Participate in multidisciplinary case coordination with Project HEAL partners.
• Support continuity of care during the stabilization period following discharge.
• Maintain documentation consistent with Claratel Behavioral Health policies and regulations.
• Complete data entry into HMIS (ClientTrack) system.
