Job summary

Claratel Behavioral Health is seeking a Targeted Case Manager to support adults with behavioral health challenges in DeKalb County, GA. You will conduct community-based needs assessments, coordinate care across providers, and help clients access housing, healthcare, and essential services.

The role emphasizes advocacy, ethical communication, and collaboration with interdisciplinary teams to create recovery-focused plans and improve client stability in the community.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, or related field.
• Proficient with electronic medical records and telehealth platforms.
• Strong care coordination skills to connect clients with internal and external services.
• Valid Georgia Driver’s License and ability to pass a defensive driving course.

Responsibilities

• Complete community-based needs assessments for individuals on caseload.
• Provide case management and support services aligned with Individual Recovery Plans (IRP).
• Facilitate care coordination across service providers and support networks.
• Participate in interdisciplinary treatment team meetings to inform care strategies.
• Perform weekly outreach to clients and maintain high-quality documentation.
• Advocate for clients and help them access housing, healthcare, and community resources.
• Provide transportation for clients to appointments and events.
• Communicate clearly with agency staff, families, and community partners.
