Job summary

Claratel Behavioral Health seeks a Child & Adolescent Case Manager/Community Support Individual to support recovery-focused services in the community. The CSI will assist with care coordination, participate in meetings, and help families navigate resources within the program.

The role requires a bachelor’s degree in a related field and at least two years of behavioral health case management experience, with strong tech skills and transportation to work. Weekend/shift hours may apply as needed.

Qualifications

• Bachelor’s degree in a related field (counseling, criminal justice, social work)
• At least two years’ experience in a behavioral health setting providing case management/community support services
• Proficient in technology to use EMRs, computers, telehealth systems, and cell phones
• Care coordination to link clients with resources and services
• Must have transportation to and from work
• Pre-employment drug screening may be required
• FBI Criminal History Record Check and ability to challenge records

Responsibilities

• Completes community needs and clinical assessments for all caseloads
• Participates in interdisciplinary treatment team meetings
• Provides ongoing case management and skill-building aligned with treatment plans
• Delivers timely documentation for all service contacts
• Meets with the Program Manager for professional growth and admin duties
• Outreaches to clients at least twice monthly
• Facilitates communication between community providers for clients
• Maintains safety and health communications with the Program Manager
• Transports participants on community outings
• Performs additional duties as assigned by Program Manager/Director
