Responsibilities

• Conduct intake and develop ISP addressing housing goals and income.
• Provide referrals, linkages, information, and support to resources.
• Complete program-specific assessments (e.g. VI-SPDAT).
• Evaluate strengths and challenges; conduct a 90-day review.
• Maintain documentation and HIPAA compliance.
• Develop therapeutic relationships and provide crisis intervention.
• Use motivational interviewing and trauma-informed care in service delivery.
• Maintain up-to-date knowledge of community resources for wrap-around services.
