Job summary

PATH (People Assisting The Homeless) is seeking a Case Manager for the Site Based Housing team at our San Jose office to assist clients in breaking the cycle of homelessness through individualized supports and housing-focused services.

Responsibilities include intake, ISP development, referrals, assessments, confidentiality, crisis intervention, and coordinating wrap-around services while using motivational interviewing and trauma-informed care to help clients achieve housing stability and

Qualifications

• Two years of experience in healthcare, case management, homeless services or a related field.
• All levels of education and experience are welcome.
• Mandatory requirements: CPR/First Aid, tuberculosis test, background screening, and drug test.

Responsibilities

• Conduct intake and individualized needs assessment and develop ISP addressing housing goals, income, and other personal goals.
• Provide referrals, linkages, information, and support to resources to achieve ISP goals.
• Complete program-specific assessments (e.g. VI-SPDAT).
• Evaluate strengths and challenges and conduct a 90-day review.
• Maintain documentation standards, including case notes and a complete participant file.
• Maintain confidentiality per HIPAA and guidelines.
• Develop therapeutic relationships with clients and partners to support housing goals.
• Provide crisis intervention and risk assessment focused on safety and self-care.
• Use motivational interviewing and trauma-informed care in service delivery.
• Maintain knowledge of community resources to provide wrap-around services.
