Job summary

Bucks County Housing Group (BCHG) is seeking a Community-Based Program Case Manager to support individuals and families experiencing homelessness or housing instability. The role delivers individualized, client-centered case management in clients’ homes and across the community, focusing on assessment, goal planning, housing stabilization, and progress monitoring.

Responsibilities include regular client meetings, collaboration with partners, and ensuring accurate documentation.

Qualifications

• Bachelor’s in Social Work or related field with 2+ years CM experience, OR Associate’s with 4 years CM experience.
• Experience with homelessness or housing instability preferred.
• Strong client-centered and trauma-informed approach required.

Responsibilities

• Conduct regular case management meetings with clients, generally weekly in-home when possible.
• Identify needs, barriers, and goals; develop strength-based service plans.
• Assist clients in locating, securing, and maintaining housing.
• Maintain HMIS documentation and program records accurately.
