Job summary

The role focuses on connecting residents with resources, navigating barriers, and sustaining housing within a clearly defined scope. You will work across multiple functions to ensure supportive services are delivered effectively, escalate high-risk cases, and maintain thorough documentation in HMIS.

Responsibilities

• Serve as resident's primary point of support for housing stability, acting as the connective link between the resident and the property/case-management systems around them.
• Support residents whenever a need arises that affects their housing situation.
• Handle resident questions related to supportive services.
• Help residents understand lease terms and expectations; support resident understanding without directly enforcing the lease.
• Help residents address underlying barriers connected to lease violations, and log related conversations in HMIS.
• Support residents with resources and transition planning when facing eviction or non-renewal.
• Address behavioral or support-related barriers through individualized case planning.
• Help residents identify resources or income solutions to address rent and delinquency concerns.
