Qualifications

• Bachelor's degree in social work or related field, or equivalent case-management experience.
• Experience with social service agencies and grant programs.
• Strong verbal and written communication skills.
• Effective time management and ability to manage a client caseload.
• Ability to work with diverse populations and disability considerations.
• Valid Oklahoma driver’s license with good driving record.

Responsibilities

• Oversee the overall management and direction of assigned clients.
• Coordinate services to close gaps and ensure quality individualized support.
• Develop and monitor Individual Action Plans (IAP) for clients.
• Collaborate with staff and partner agencies; coordinate resources.
• Ensure data entry is complete and accurate in the agency database.
• Monitor and report on client progress and outcomes.
• Travel as needed to meet clients and access resources.
• Maintain required documentation and support agency policy.
