Job summary

Homeless Alliance in Oklahoma City is seeking a Case Manager to support clients under the HOPWA grant, addressing housing needs and providing home-based case management, referrals, and advocacy. The role includes monitoring progress, linking to resources, and tracking outcomes for participants.

The position requires strong advocacy, excellent communication, and the ability to document in HMIS; some travel and flexible hours may be required to meet client needs.

Qualifications

• Undergraduate degree in social work or behavioral science, preferred.
• Two plus years of previous experience in adult case management, preferred.
• Computer literate; experienced with Microsoft Office (Word, Excel), Outlook, Internet.

Responsibilities

• Assists clients in the development of a Housing Stability Plan.
• Explain and interpret federal and local HOPWA regulations and rules to clients.
• Provides field based/mobile case management services using evidence-based practices to persons experiencing long term homelessness.
• Provides Rapid Re-Housing and/or Rapid Exit Case Management.
• Maintains precise documentation of case management services in HMIS and client files.
