Qualifications

• Bachelor’s degree in social work or human services related field is required or 6 months encampment rehousing case management experience.
• Fluency in Spanish and English preferred
• Strong verbal and written communication, listening, and presentation skills.

Responsibilities

• Provide in-person, home-based case management services.
• Enroll clients in HMIS and assist with unit/voucher applications.
• Assist with move-ins and transportation as needed.
• Coordinate with outreach teams and community partners to stabilize clients.
• Document case notes thoroughly in HMIS and client records.
• Participate in encampment huddles and stabilization work groups.
• Act as liaison with Coordinated Entry and behavioral health partners.
