Qualifikationen

• Bachelor’s Degree or equivalent with at least 4 years in case management or related field.
• Two years of relevant experience in homelessness, mental health, or substance use.
• Bi-lingual English and Spanish required.

Aufgaben

• Conduct intakes, assessments, service plans, reassessments, and updates.
• Provide referrals, advocacy, and housing inspections as needed.
• Enter client services into Client Track and maintain audit-ready charts.
• Escort clients to appointments and connect to medical and social services.
• Monitor progress and document monthly projections and reports.
