Job summary

Children's Rescue Fund in New York, NY seeks a dedicated case manager to provide direct services including assessment, counseling, crisis intervention, referral, and advocacy to help families secure permanent housing.

Responsibilities include using DHS CARES and coordinating with partner agencies, documenting progress, supporting families with life skills, and ensuring timely, compliant service delivery in a dynamic social service environment. Travel across NYC and on-call duties may apply.

Qualifications

• Bachelor's degree is required.
• Experience facilitating groups for homeless adults and knowledge of substance abuse treatment.
• Bilingual language ability highly desirable.
• Excellent written and verbal communication skills.
• Excellent interpersonal skills and computer literacy in Microsoft Office.

Responsibilities

• Provide direct case management including assessment, counseling, crisis intervention and advocacy to help families achieve permanent housing.
• Complete intakes, service plans, progress notes and incident reports using CARES.
• Coordinate services with DHS, ACS, Probation, DHS CARES and other agencies as needed.
• Provide advocacy and referrals to ensure clients receive benefits and services for a housing transition.
• Facilitate group sessions and document client progress to support self-sufficiency.
• Ensure timely, compliant service delivery and maintain accurate records.
• Travel across NYC, may be on-call one week per quarter, and participate in staff development.
