Job summary

The People Concern in Lancaster, CA is seeking a Temporary Case Manager to support housing stability for clients experiencing homelessness. The role is on-site at Kensington Campus, Monday–Friday, 8:00am–4:30pm, with benefits and a focus on harm-reduction and trauma-informed care.

You will manage a caseload, coordinate with LA County DMH and DHS teams, assist with benefits, and transport clients as needed.

Qualifications

• High School Diploma or GED required.
• Bachelor's degree preferred, or 2+ years' experience with homeless individuals.
• Valid CA driver’s license, reliable car, auto insurance, and a clean driving record.
• Willing and able to transport clients in your personal vehicle.
• Ability to work in a harm-reduction model with clients facing multiple barriers.

Responsibilities

• Maintain a caseload of approximately 20 high-acuity clients (up to 40 for low acuity).
• Develop relationships with clients focused on independence and improved health.
• Coordinate with LA County DMH and DHS programs to align treatment teams.
• Engage clients with an approach emphasizing choice, harm reduction, and trauma-informed care.
• Assist in screening, intake and enrollment including orientation to policies and goals.
• Complete biopsychosocial and functional needs assessments to guide treatment planning.
• Prepare quarterly collaborative goal plans to improve quality of life and health outcomes.
• Write and maintain progress notes in electronic databases and ensure chart accuracy.
• Coordinate transportation and follow-up services to improve access to care.
• Directly transport clients in personal vehicle to agencies in the greater LA area.
• Teach life skills such as budgeting, housekeeping, meal prep, appointment scheduling and health maintenance.
• Communicate with property managers and third-party partners.
• Participate in weekly staff meetings and trainings; adapt workload as funder expectations shift.
