Job summary

Avenue 360 is seeking a Continuum of Care Housing Case Manager to provide permanent supportive housing program participants with case management and supportive services to maintain housing stability and prevent homelessness.

This role requires coordinating with internal teams, property owners, and community partners, conducting on-site visits and home visits, and documenting progress in HMIS with clear, timely communication.

Qualifications

• Bachelor’s degree or commensurate experience; undergraduate degree in social work or closely related field preferred.
• At least 2 years of case management or customer service experience required.
• 2 years of housing experience with local, state, and/or federal programs preferred.
• 2 years of experience working with individuals and/or families experiencing homelessness preferred.

Responsibilities

• Assess participants’ eligibility and enrollment documentation.
• Enroll clients into housing programs via Coordinated Access System referrals.
• Assist participants in securing housing based on household size and geographic preferences.
• Provide case management through on-site visits, phone consultations, and home visits.
• Develop individualized service plans addressing residential stability, income, education, and health-related goals.
• Demonstrate knowledge of community resources and coordinate linkages to services.
• Develop and maintain relationships with property owners and property management.
• Assist clients with housing search and rental applications; educate on Fair Housing laws and tenant responsibilities.
• Determine rent subsidy amounts per federal guidelines; conduct HQS inspections before move-in and annually.
• Secure all required documentation; prepare and submit rent/utility assistance requests timely.
• Maintain HMIS data compliance; manage client information accurately.
• Maintain professionalism in emotionally charged situations; perform other duties as assigned.
