Job summary

Avenue360 Health and Wellness in Houston seeks a Housing Case Manager to deliver case management and supportive services within the Continuum of Care program, helping participants achieve stable housing and avoid homelessness. You will coordinate with partners, document services, and support clients through on-site visits and home visits.

Responsibilities include assessing eligibility, developing service plans, coordinating referrals, managing data in HMIS, and ensuring compliance with housing

Qualifications

• Bachelor's degree or commensurate experience.
• 2 years of case management or customer service experience required.
• 2 years of recent housing experience with local/state/federal programs preferred.
• 2 years of experience working with individuals/families experiencing homelessness preferred.
• Bilingual (English/Spanish) desired.

Responsibilities

• Assess participants' eligibility and enrollment documentation.
• Enroll clients into housing programs via Coordinated Access System referrals.
• Assist participants in securing housing based on household size and preferences.
• Provide case management through on-site visits, phone consultations, and home visits.
• Develop individualized service plans addressing stability, income, education, and health goals.
• Demonstrate knowledge of community resources and coordinate linkages.
