Job summary

Avenue360 Health and Wellness in Houston, TX seeks a Housing Case Manager to provide permanent supportive housing program case management and supportive services to help maintain housing stability and prevent homelessness. Qualifications

• Bachelor’s degree or commensurate experience is required.
• 2 years of case management or customer service experience required.
• 2 years of housing experience with local/state/federal programs preferred.
• 2 years of experience working with individuals/families experiencing homelessness preferred.

Responsibilities

• Assess participants’ eligibility and enrollment documentation.
• Enroll clients into housing programs via Coordinated Access System referrals.
• Assist participants in securing housing based on household size and preferences.
• Provide case management through on-site visits, calls, and home visits.
• Develop individualized service plans addressing housing stability, income, education, and health goals.
• Coordinate linkages to community resources and services.
• Maintain data in HMIS and ensure accurate client information.
• Communicate with property owners and management; oversee rent subsidies and HQS inspections.
