Job summary

Avenue 360 Health and Wellness is seeking a Continuum of Care Housing Case Manager to deliver case management and supportive services for housing stability. The role involves coordination with internal teams, property owners, and community partners to support participants in securing and maintaining housing.

The position emphasizes documentation, evaluation of eligibility, and strong client-centered service planning to address residential stability and health-related goals.

Qualifications

• Bachelor’s degree or commensurate experience; undergraduate degree in social work or closely related field preferred.
• At least 2 years of case management or customer service experience required.
• 2 years of recent housing experience with local, state, and/or federal programs preferred.
• 2 years of experience working with individuals and/or families experiencing homelessness preferred.

Responsibilities

• Assess eligibility and obtain enrollment documentation.
• Accept Coordinated Access System referrals and enroll clients.
• Help participants secure housing based on household size and needs.
• Provide case management through on-site visits, calls, and home visits.
• Develop individualized service plans addressing stability, income, education, and health goals.
• Coordinate linkages to community resources and services.
