Qualifications

• Bachelor’s degree in social work, counseling, psychology, or related field.
• Experience/knowledge of homelessness and/or marginalized communities preferred.

Responsibilities

• Providing support services to individuals and families active in Homefull programs (prevention, shelter rapid rehousing, permanent supportive housing).
• Developing and working toward individualized service plans (ISPs) that include housing stability and other goals.
• Completing high quality clinical documentation as part of Collaborative Documentation efforts.
• Maintaining client case records per agency and HMIS policies and procedures.
• Contributing to performance outcomes including client contacts, mental health support, housing stability, income, benefits, and recidivism reduction.
• Conducting home visits, community meetings, transportation, site-based sessions, and telehealth as needed.
• Submitting reports and responding to information requests.
• Meeting productivity goals based on caseload.
• Participating in ongoing education to develop professional skills and achieve QMHS status via Homefull training.
