Job summary

Renaissance Social Services, Inc. in Chicago seeks an experienced Housing Specialist to join the Rapid Rehousing Team and provide assessments and intensive case management for homeless clients placed in scattered-site apartments across the city.

The role aims to ensure clients gain income and housing stability within 9 to 12 months or transfer to Permanent Supportive Housing, with up to 50% travel, day shift, and a salary of $43,000–$44,000.

Qualifications

• Must meet Illinois criteria for Mental Health Professional (MHP) via one of the pathways.
• 3–5 years of clinical experience, preferably in case management or direct community-based service.
• Proven experience with chronic mental illness and substance use disorders with Harm Reduction.
• Ability to relate to clients, families, staff and partner agencies.
• Strong computer literacy and EHR documentation.

Responsibilities

• Assess clients’ mental health to determine Rapid Rehousing (RRH) or Permanent Supportive Housing (PSH).
• Collaborate with Housing Liaisons to move homeless individuals/families into housing quickly.
• Provide intensive case management in the community over 9–12 months to ensure stability.
• Identify needs, strengths, and barriers and support problem-solving.
• Coordinate services and advocacy with community agencies and housing sites.
