Job summary

Bronx Family Network Inc in Yonkers, NY is seeking a full-time Case Manager to provide comprehensive support to shelter residents, including needs assessment, case management, and crisis intervention, connecting clients with community resources and advocating for their well-being.

You will maintain documentation in CARES, manage a caseload of 20-25, collaborate with shelter staff, and help residents pursue housing and services while adhering to DHS and OTDA regulations.

Qualifications

• Bachelor’s degree in a relevant field (social work, psychology or human services).
• Minimum 2 years of case management experience.
• Experience with homeless or underserved populations.
• Familiarity with community resources, housing resources and benefits; CARES experience preferred.

Responsibilities

• Conduct assessments to determine client needs and develop ILP, with ongoing case management.
• Lead weekly/bi-weekly ILP sessions and document in CARES within 48 hours.
• Perform unit inspections and upload documentation in CARES.
• Assist clients with housing searches, jobs, and referrals; document in CARES.
• Maintain accurate client data, intake forms, and progress notes per policy.
• Connect clients with housing, healthcare, and social services; advocate for needs.
• Educate clients on budgeting, tenant relations, and healthy living practices.
• Adhere to shelter policies, DHS and OTDA regulations; coordinate care with staff.
• Manage caseload of 20-25 cases with up-to-date records.
• Report problems to shelter director and perform other duties as assigned.
