Job summary

The Salvation Army Northern Division Anoka 360 Life Center in Blaine, MN seeks a Case Manager to provide strength-based, person-centered case management for individuals and families experiencing homelessness or housing instability. This role requires engaging participants, assessing needs, and coordinating access to housing resources.

Qualifications

• Bachelor’s degree in a relevant field. An equivalent combination of education and experience may be considered in lieu of a degree.
• Must be licensed by the state board if the degree is in social work.
• Previous experience in social services required.
• Must pass background checks, possess a valid driver’s license, and meet MVR requirements.
• Strong interpersonal and communication skills.
• Maintains confidentiality and exercises sound judgment when handling sensitive information.
• Works with integrity, valuing honesty and diligence.
• Excellent organizational skills and attention to detail.
• Proficient with PC, Microsoft Office, databases, and standard office equipment.

Responsibilities

• Provide strength-based, person-centered case management to individuals and families experiencing homelessness or housing instability.
• Identify needs, navigate housing and community resources, access available assistance, and develop goals to support housing stability and self-sufficiency.
• Complete participant assessments, maintain required documentation, and coordinate services and referrals.
• Collaborate with community partners to help individuals and families obtain and maintain stable housing.
• Perform administrative tasks and ensure documentation accuracy.
