Care-For-the-Homeless- is seeking a Senior Care Coordinator to oversee client services and supervise a team of Care Coordinators, Enrollment Specialists, and Housing Specialists. The role emphasizes HUD compliance, housing navigation, and quality improvement across NYC and surrounding areas.

The ideal candidate has a strong background in social services, case management, or a related field, with leadership experience and a passion for helping vulnerable populations.

Qualifications

• Bachelor’s degree or higher in a related field preferred.
• Minimum 5 years of experience in social services or case management.
• 3–5 years of management/leadership experience in healthcare or related field.
• Knowledge of HUD regulations and HUD-funded programs preferred.

Responsibilities

• Oversee coordination of client services and supervise Care Coordinators, Enrollment Specialists, and Housing Specialists.
• Conduct assessments and rapid re-housing intakes; maintain accurate case notes and documentation.
• Lead home visits and office-based case management to secure suitable housing for clients.
• Collaborate with landlords, brokers, and community housing partners to identify housing options.
• Ensure compliance with HUD regulations and drive quality improvement initiatives.
