Job summary

Care For the Homeless is seeking a Senior Care Coordinator in New York to oversee client services, coordinate a team of Care Coordinators, Enrollment Specialists, and Housing Specialists, and ensure HUD compliance across housing programs.

The role includes leading quality improvements, managing up to 25+ clients, and engaging in field/home visits to secure suitable housing for vulnerable populations in NYC and surrounding areas.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Public Administration, or related field (preferred).
• Minimum of 5 years of experience in social services, case management, or a similar role.
• 3-5 years management and progressive leadership experience in healthcare or related field.
• Knowledge of HUD regulations and experience with HUD-funded programs (preferred).

Responsibilities

• Conduct assessments and program intakes for rapid re-housing services.
• Ensure data is accurate and complete and that all required accompanying documentation is collected.
• Update case notes in electronic case management system in a timely manner.
• Participate in the intake and orientation process of new program participants.
• Provide direct case management services in the office and on field/home visits.
• Advocate for housing and assist clients to find suitable housing across NYC and surrounding areas.
• Work with community housing specialists to identify suitable housing and conduct housing inspections.
• Mediate landlord-tenant disputes and maintain positive landlord relationships.
• Develop and update housing stability plans to obtain or maintain permanent housing.
• Manage a caseload of up to 25+ clients and provide oversight to Care Coordinators, Enrollment Specialists, and Housing Specialists.
