Job summary

Economic Opportunity Council of Suffolk is seeking a Senior Case Coordinator to provide case management and housing support to participants experiencing homelessness. You will help households obtain and maintain permanent housing, coordinate services, and mentor junior staff.

Responsibilities include serving as a liaison with housing providers, assisting with benefits access, and tracking participant progress through HMIS.

Qualifications

• Strong verbal and written communication skills, organizational skills, and attention to detail.
• Works well in a team, builds respectful relationships with participants.
• Proficiency with Word, Excel, PowerPoint, Outlook and HMIS; ability to learn agency systems.

Responsibilities

• Support participants in obtaining and maintaining permanent housing through ongoing services that promote housing stability and self-sufficiency.
• Assess participant needs, strengths, income, and housing barriers; develop stability plans and budgets.
• Coordinate services, referrals to housing providers, treatment services, employment programs, and community agencies.
• Assist participants in obtaining employment and increasing income through job training and benefits.
• Mentor Case Managers and provide guidance on complex cases; assist with training of new staff as assigned.
